ABSTRACT

Objective To investigate the effect of bronchofibroscopic bronchoalveolar lavage on serum inflammatory cytokines in the patients with severe ventilator-associated pneumonia(VAP) .Methods 68 cases of severe VAP were divided into the control group and observation group according to the random number table method ,34 cases in each group .The control group used conventional alveolar lavage plus sputum suction treatment ,while the observation group adopted bronchofibroscopic bronchoalveolar lavage plus sputum suction treatment .The lung infection control time ,mechanical ventilation treatment time ,time of body temperature recovery to normal ,pulmonary infection score(CPIS) ,white blood cell(WBC) count ,respiratory function ,blood gas indexes and serum inflammatory factors levels were compared between the two groups .Results The lung infection control time ,mechanical ventilation treatment time ,recovery time of normal temperature ,blood gas indexes and serum inflammatory factors levels after treatment in the observation group were significantly superior to those in the control group (P<0 .05) .Conclusion Bronchofibroscopic bronchoalveolar lavage can significantly improve the respiratory function and blood gas indexes ,reduces the serum inflammatory factors levels in the patients with severe VAP .

ABSTRACT

OBJECTIVE:To provide reference for rational use of second class of psychotropic drugs in the clinic. METHODS:The second class of psychotropic drugs in outpatient department of our hospital during 2012-2015 were analyzed retrospectively in respects of the number of common drugs prescription,distribution of gender/age/disease diagnosis,DDDs and DUI,etc. RE-SULTS:During 2012-2015,second class of psychotropic drugs in outpatient department of our hospital accounted for 25.09%of to-tal prescription amout. The drugs with high use frequency were alprazolam,clonazepam,lorazepam and estazolam,accounting for 93.10% of total prescription amount of second class of psychotropic drugs. The proportion of male to female was 1∶1.50;the pa-tients aged 19-35 years old took up the biggest proportion,accounting for 34.62%. The proportion of schizophrenia prescription was the highest,accounting for 45.17%. DDDs of alprazolam was the highest,being 742 141.67;its DUI was 1.075;DUI of oth-er drugs was lower than 1.0. CONCLUSIONS:The use of second class of psychotropic drugs is basically reasonable in our hospi-tal. Guiding principles of Clinical Application of Psychotropic Drugs should be implemented strictly and continously to avoid abuse of second class of psychotropic drugs.

ABSTRACT

Objective To investigate characteristics of pathogen distribution and drug resistance,and to analyze risk factors of the infection in patients with vaginitis in Zhangjiakou area.Methods Vaginal secretion specimen from 368 patients with vaginitis were collected and then processed by germiculture,identification and drug sensitivity tests.Risk factors of infection were identified among age,marriage history,first sexual intercourse age,education,childbearing history,abortion history,contraception,householding et al by single factor x2 analysis and Logistic regression analysis.Results Blastomyces albicans,ureaplasma urealyticum,escherichia coli,enterococcus faecalis and gardner vaginal bacteria listed top 5 of composition of infection rate in patients with vaginitis.Blastomyces albicans was sensitive to multiple antifungal agent; escherichia coli and gardner vaginal bacteria were resistant to multiple antibiotics but sensitive to imipenem;ureaplasma urealyticum was resistant to multiple antibiotics but sensitive to josamycin and clarithromycin;enterococcus faecalis was strongly resisted to SMZ and erythromycin.Age ranging from 31 ~40y,application of IUD,positive abortion history and first sex intercourse before 23y were identified as risk factors for infection in patients with vaginitis.Conclusion Infection characteristics and risk factors could be utilized as indicators in considering treatment programs for patients with vaginitis.

ABSTRACT

Objective To investigate the electrocardiographic(ECG) characteristics and assess the safety of radiofrequency catheter ablation (RFCA) of ventricular tachyeardia (VT), and frequent ventricular premature con- tractious(VPCs) originating from left ventricular outflow tract(LVOT). Methods Twelve-lead ECG analysis and RFCA were performed in 9 patient with VT or VPCs originating from LVOT. Results The sites to be successfully ablated were in left valsalva sinus in 6 patients,and subaortic valve in 3 patient. ECG revealed tall R wave in leads Ⅱ, Ⅲ ,aVF, V5 and V6. QS morphology in both leads aVR and aVL,QS morphology in lead Ⅰ in most cases. R wave in lead Ⅰ was relative tall, the R/S wave amplitude in lead V1 was over 0.62 ,and precordia R wave transition was irregular. All 9 patients were successfully ablated. No death or recurrenced case during follow-up. Conclusion Ventricular taehycardia and frequent ventricular premature rcontractions originating from left ventricutar outflow tract could be known by some specific ECG characteristics and could be radically treated safely by RFCA.
